Job Title: Social Worker or Social Worker Assistant

Job Summary:

We are seeking a qualified Social Worker or Social Worker Assistant to join our Adult Day Health Care Center team. The successful candidate will provide assistance, support and counseling to patients and their families. The Social Worker will act as a member of the multidisciplinary team and will collaborate with other health care professionals to ensure quality care is delivered.

Responsibilities:

• Provide individual and group counseling for patients and their families
• Develop and implement therapeutic treatment plans
• Monitor patient progress and modify treatment plans as needed
• Conduct assessments and provide crisis intervention as needed
• Coordinate and/or provide referrals to community services and resources
• Participate in multidisciplinary team meetings
• Maintain current knowledge of available resources
• Prepare reports and documentation
• Provide patient and family education

Requirements:

• Master’s degree in social work or related field
• Licensed or certified as a Social Worker (if applicable)
• Excellent communication, problem-solving, and organizational skills
